Client Card

The K9s Only Client Card is required for all clients and every service. If you have an appointment at K9s Only please print and fill out the form. You may fax the form to 818-342-3120 ahead of time; however, please call to confirm receipt of the fax.

Medication Intake Form

Any dog that will need medication or supplements (prescribed or over-the-counter) during his stay at K9s Only will need to have a Medication Intake Form on file. Each form allows for a description of two medications; if your dog requires more than two please feel free to use as many forms as necessary. This information allows us to take the best possible care of your dog so details are appreciated!

Personal Items Form

Any dog staying at K9s Only with personal items will need to have a Personal Items Form on file. The details on this form allow us to keep track of your dog’s personal belongings.

Directions

We are located just east of the 405 freeway between Olympic and Santa Monica. Take the Olympic/Pico exit and travel north on Sawtelle. Turn right on Olympic then left on Cotner. Our building is on the right hand side of the street. You may park on the street on in the lot in front of the building.

Directions

We are located approximately 1/2 mile north of the 101 freeway. Take the Reseda Blvd. exit and travel north (make a right if you are coming from Los Angeles). Travel approximately 1/2 mile to Topham Street (just past Oxnard St. and the Metro Orange Bus Line), turn left.

Our building is on the right hand side of the street. You may park on the street; in the back, behind K9s Only; or across the street in the Orange Line bus parking lot.
